Licensed Practical Nurse (LPN)
Location: Pleasant Valley, NY
Schedule: Full-Time, 32-40 hours per week
Pay: $28.00 per hour
Shift: Day shift (7am-3pm)
About the Role
The Licensed Practical Nurse (LPN) at provides essential nursing support to individuals with intellectual and developmental disabilities in a community-based setting. This role focuses on consistency, coordination, and quality care—not hospital-floor stress or revolving-door patients.
You'll work under the direction of an RN while collaborating closely with direct support professionals (2-3 per shift), supervisors, families, and healthcare providers to ensure safe, compassionate, and person-centered care.
What You'll Do
- Provide nursing services to adults with intellectual and developmental disabilities (approx. 10 people in your care per shift)
- Administer medications and treatments as ordered
- Monitor health status and report changes to the RN and care team
- Assist with implementation of individualized healthcare plans
- Maintain accurate medical records and documentation
- Support medical appointments, follow-ups, and continuity of care
- Educate and guide direct care staff on basic health and medication protocols
- Ensure compliance with NYS regulations and agency policies
Experience We're Looking For
- Active NYS Licensed Practical Nurse (LPN) license (required)
- Strong organizational skills and attention to detail
- Ability to work independently and as part of a team
- Clear communication and a calm, professional approach
- Experience in IDD, community health, or long-term care is a plus—but not required
- A genuine interest in supporting individuals to live healthy, independent lives
